REaCH – One of 5 UK RCNi finalists

The REaCH Service (Responsive Education and Collaborative Health programme for Nursing Home Registrants) has been working within NHSCT incrementally from 2015. The aim of the REaCH programme is to enhance the skills set of registrant nurses in care homes so to improve the resident experience for those whose attendance at ED could potentially be avoided.

The REaCH service currently provide visible support to 57 Nursing Homes within the NHSCT, providing the homes with a range of Training, Education and Development opportunities. The team signpost Nursing Home staff to other relevant services to support proactive management of their residents’ condition. The team also engage with Acute Hospital colleagues to case find and support patients’ hospital discharge where a new training need for care home staff has been identified.
